Looking to add a database survey to our website. The database will retain the information in Access or be able to import into access. We will want the flexibility to due multiple surveys with a few changes for each one. It was also need to have some question cross tabulate based on income levels and other questions. Q1 with Q2, Q1 with Q3.. For example Q1 will have A: below 50k,B 51k-75k,etc.... we will need to change this for each survey. Q2, Q3, Q4... could have questions that will answer 1: most likely to 5: not likely.
Other questions will need to enter text and some may have multiple choice answers. Again all this needs to be able to easily edit the options for next survey. If you have suggestions or questions let me know. I need to know your estimated hours and your rate separate, not just a bid amount.
