Description: «Login» to edit the description	I need a small database developed for a healthcare company to search information based on the data inserted and output results. Also, it should be able to upload data file (3gigs file) every month by an user. Lastly, it should have a mechanism to add notes to searched data. In Summary a) Database queries and tables (preferabbly webbased form) b) Uploading mechanism ( to upload data file ) c) Front End 1) Login page 2) add/Edit user screen 3) list page ( shows all therapist, OT, etc ) 4) Search page ( by zip code, first name, last name, id ) 5) Notes or history
