Description: «Login» to edit the description	Application platform: Preferably Adobe Air Description: A standalone application that will act as a form filler for industrial safety accident report forms. Basics: Every state in the United States has a different form that employers must use when there is an industrial safety accident. These forms are openly and publicly available by PDF (and I will supply the list of links). Some of these forms can be confusing for everyday employees, so I want to create an application that will ask the appropriate questions (who, what, where, what part of body, etc.) and then asks what state to print the form for (i.e. accident happened in michigan, california, nebraska, etc.) and fill out the appropriate fields on the PDF form and print it out. Advanced: Some forms ask for body codes and accident codes, which the normal everyday employee doesn't know. I want to make it very easy to fill in the right body code (ex: click on a picture of a persons' head, the image will enlarge around the head, and the user can click on the left ear... this puts code ZY in the corresponding body code on the form). For the accident codes, we might just have to utilize a drop down list or something. I am pretty flexible about the overall look of the application, as long as it is user friendly and contains my company's logo through out. This application will be used as free-ware and advertising for my company. I am a small sole-proprietorship based out of Michigan.
